I hereby: Release and waive Faith Bible Church, its directors, volunteers, and agents from any and all liability, claims, or demands for accidental personal injury, sickness, death, or property damage that may be incurred by my child or myself during VBS activities. I agree to assume all risks of accidental personal injury, sickness, death, or property damage as a result of my child's participation in VBS. I consent to any medical treatment deemed necessary by Faith Bible Church staff in case of injury or illness.
